Forms can be used for both internal people (Glasscubes users, whether staff or not), or external people if you just need to run a relatively quick poll that doesn't need sophisticated analysis of the results. On the left hand side of the page, you will see a tab that says ''Forms'' and that will allow you to create forms for people to fill in and complete. 


You can either create a form from scratch, or link an already made form to a Workspace that doesn't have it. (i.e linking a form from RCOphth Central to RCOphth department name) 

When you click ''Link form'', it will bring up another source/Workspace that you can get the form from. So in the example shown below, it will link the form that had been already made in RCOphth Central to the RCOphth IT page. Then click ''Link form''. 

You can create your own form by clicking on ''Create form'' and it will take you to a page which will have the preview of how it will look and a side bar that has different features you would like to add to the form you're creating. And you can drag and drop the way you would like your form to be answered by dragging and dropping from the left side of the menu to the middle of the page where it says ''Drag and drop fields into your form ''.

Then you can type in whatever question you would like to have on there and if you would like it to be multiple choice or single choice or if you would like a written answer, you can also add an option for a signature to be added. You can also provide the option for the submissions to be anonymous. See screenshot below for a test form.



Then you can click ''preview'' at the top right corner to see how the form would look like before you complete it and send it off for people to fill in.  See screenshot of the preview below.

Then when you want to carry on with publishing it, you can go back to the editing page and click on ''next'' at the top right corner which will then take you back to the editing page and you can click ''next'' once you're happy with the preview. The screenshot below is the page that will pop up when you click ''next'' 


You can edit the message you want to go along with the form when you send it to the people you want to send it to, and once you've finished filling out this section, you click next and it will take you to the page which will ask you how you want to share the page.


Then once you've completed filling that out, you can publish your form to be officially filled in by the members you have sent it to.
